Diploma Mailing Request Form

The mailing fee is $20; however, we will mail diplomas or certificates at no charge for School of Nursing students (MEPN, DNP, Supplemental Studies in Advanced Practice in Nursing, Nursing MS, Nursing PhD, and Sociology PhD) who graduated in spring 2006 or later, Master of Translational Medicine students who graduated in spring 2020 or later, School of Pharmacy students who graduated between fall 2006 and fall 2012, and Equity in Brain Health students.

We ship diplomas destined to U.S. addresses by U.S. Postal Service First-Class Mail. A signature will be required upon delivery.

If we mail your diploma and it is returned as undeliverable, the fee to mail it again will be $25.

Placing your order via Check, Cashier's Check, or Money Order?

Please include the following information:

Your name when you graduated from UCSF and, if different, your current name
Program or school attended
Degree
Date of graduation
Address to send diploma or certificate (a signature is required upon delivery)
Your signature

Please sign your request and enclose a check, money order, or cashier's check payable to UC Regents for the total fee or provide your Visa, MasterCard, Discover, or American Express information, including your card number, the expiration date, and the three-digit code from the signature block on the back of the card. For credit card orders only, you may fax your request to (415) 476-9690. We cannot accept credit card information by e-mail.
