Diplomas & Certificates
The name on your diploma or certificate will be your name in university records as of your last quarter of registration.
To change your name in university records and on your diploma or certificate, please submit a Petition for Name Change to the Office of the Registrar during the first two weeks of your final quarter of enrollment. This form is available at our office in MU-200W.
If you have already graduated, submit a Petition for Name Change After Graduation.

Doctor of Medicine, Doctor of Dental Surgery, Doctor of Pharmacy, Doctor of Physical Therapy diplomas: 11" X 17"
All other diplomas and certificates: 8.5" X 11"
