How To Obtain Your Diploma
Diplomas and certificates are available approximately four months after you graduate. Unclaimed diplomas and certificates may be destroyed after five years. You can pick up your diploma or certificate in person, or we can mail it to you.
If your diploma is lost or damaged, or if you changed your name and need to order a diploma in your new name, please refer to the diploma reissue process.
In Person
Please bring a picture ID to MU-200W. If you wish to permit a third party to pick up your diploma or certificate, you must provide a signed and dated statement indicating the name of the person whom you are permitting to pick up your diploma or certificate. The third party must bring his/her picture ID, a copy of your picture ID, and a copy of your signed and dated statement.
The mailing fee is $20; however, we will mail diplomas at no charge for School of Nursing students who graduated between spring 2006 and the present and School of Pharmacy students who graduated between fall 2006 and the present. Please use our Diploma Mailing Form. Alternatively, you may send us a written request to mail your diploma or certificate.
If we mail your diploma and it is returned as undeliverable, the fee to mail it again will be $25.
Please include the following information:
- • Your name when you graduated from UCSF and, if different, your current name
- • Program or school attended
- • Degree
- • Date of graduation
- • Address to send diploma or certificate (diplomas and certificates are shipped via the U.S. Postal Service, and a signature upon delivery is required)
- • Your signature
Please sign your request and enclose a check, money order, or cashier's check payable to UC Regents for the total fee or provide your Visa, MasterCard, Discover, or American Express information on the Diploma Mailing Form. If you are paying by credit card and do not use the Diploma Mailing Form, please provide your card number, the expiration date, and the three-digit code from the signature block on the back of the card. For credit card orders only, you may fax your request to 415-476-9690. We cannot accept credit card information by e-mail.
